They knew that the store model that has made them the number one retailer in the world was not going to work within Chinese culture, but just how much different would they have to be from the cookie-cutter stores that are found all across the United States? In order to dominate the retail market in China as successfully as they have in other markets across the world, dynamic change and the ability and willingness to work with the Chinese government would be crucial to their success in China. Faced with the strict rules and regulations that would hamper their growth and general operations, Walmart had to find a way to make and keep China and her people happy. Along with all the excitement, pomp, and circumstance that Walmart would bring into China, certain blemishes would follow as well. Low wages, discrimination, and utter dominance over an area it occupies left many questions as to whether Walmart would be accepted or rejected. But the first stumbling block that Walmart would have to face upon entering China would be one of its most challenging: The Government of the People’s Republic of China (Naughton). Upon entering any foreign market, one of the biggest problems facing Walmart and its expansion is government regulation and government worry over the massive company’s monopolization and impact on local markets. Moving into China, Walmart was going to have to assure the Chinese government that they were there to give the Chinese people a better retail alternative, provide abundant employment, and help China’s economy by establishing healthy competition, all while providing the Chinese people with an increased range of products and increased product quality available at the lowest possible prices (Huffman). Walmart in the United States is a very powerful company. In other countries, however, they are not viewed the same. This became very clear when they entered into China. Walmart faced problems that couldn’t be fixed with quick solutions. Finding suppliers, dealing with unions, rigid local competition, and cooperating with a communist government are examples of such problems. Once it found solutions to the above problems, Walmart opened a Supercenter  and Sam’s Club in Shenzhen in 1996 (Foreman). Walmart China, as it is known today, actively participates in communities by providing funding for charities, education and medical care. In fact, in April of this year, Wal-Mart launched its third annual nationwide community service week: â€Å"Keeping the Traditional Virtues – Walmart Filial Piety Activity†. For a period of one week, community service for the elders of each specific community holding the event was performed. This government and community involvement is necessary for Walmart and the trust it needs to gain from the Chinese people (Walmart). When Walmart first opened its doors back in 1996, they first felt the restraints from the government while trying to put products on their shelves. As an result of Mao and the period when every village and community was designed to be self sufficient, Walmart could not set up conventional distribution centers such as are found here in the United States. Products that would move from one city to another would require the approval of local officials, a task of staggering proportions. Instead, Walmart has had to set up buyers in each community and city that it operates in to purchase items that have to be bought locally through government and state approved vendors. Because of this, there are not very many national brands that can be found inside of Walmart China. This is gradually changing as China evolves and her people become more influenced by western culture and style. An example would be with cosmetics. When Walmart China first opened, the cosmetics available would not sell. It wasn’t until demonstrations were given and the local Chinese ladies were given makeovers that the make-up started to sell. As a result, not only did the cosmetics started selling fast, Walmart was able to bring in more expensive, higher-end brands (Trunick). When Walmart China bought initial products from suppliers back in the first few years in operation, for every purchase order issued, there were twenty-one documents that went along with it, simply to comply with central and local government regulations. Each individual store had to write the purchase orders by hand and issue their own checks. There could be no  central ordering or payment systems. Hard paperwork was required, and still is today although not as much, which resulted in having to pay additional employees to process the paperwork, subjects documents to human error, and is simply inefficient. Today, although the rules are definitely felt and abided by, regulations are slowly easing up. Walmart is slowly becoming able to centralize more and more of its processes (Trunick). Walmart is a well-admired company today in China. Just as employee songs and Walmart chants can be found in sites in the United States, they can be found in the Chinese locations as well. Adjusting to operating under the People’s Republic of China wasn’t the only thing Walmart had to get used to. They also had to get used to local customs and preferences. When Sam’s club first opened, it didn’t do as well as the Sam’s clubs here. The simple fact was that the Chinese didn’t have storage available to them such as we take for granted here in the States. The Sam’s Club in Shenzhen had to evolve to fit the needs of its customers and not vice-versa if it was to succeed. It did and it has, respectfully (Huffman). In every country, city, and community around the world that Walmart operates in, they constantly strive to be an asset to the area they are in. Not only do they provide numerous jobs to the areas, including employment to disable persons, they enable families to be able to afford things that they may not otherwise be able to afford. To help villagers reduce their carbon footprint and provide hot water to their village, Walmart is supporting the installation of solar panels. They also have implemented a direct farm program that works to eliminate the amount of time fruits and vegetables take to get to the store. Not only does this help to ensure the best possible quality of produce, it helps sustain the local farms and villages who produce the goods. INTRODUCTION Pakistan as a developing country has very limited health care resources when  considering a huge population of over 170 million. We have very few tertiary care  hospitals and they are providing services to the whole country. Majority of people in  our country are poor and they are unable to afford the expenses of private hospitals,  though private hospitals are also very few. Thousands of doctors are unemployed  and still we have shortage of doctors. Majority of BHU (Basic Health Units) are  closed as majority of doctors belong to urban areas and they don’t want to work in  remote village areas. In all these situations, it is very difficult to maintain health  care throughout country. In this essay, I will take into account four conditions of  accountability for reasonableness for priority setting and resource allocation. I will  take into account these four conditions by Norman Daniels and I will consider a  tertiary care hospital scenario where I did my house job in medicine ward last year. There were majority of patients suffering from chronic liver diseases (CLD). I will  further continue this essay in discussion. DISCUSSION Before discussing the four conditions of accountability for reasonableness, I will  briefly discuss the case scenario. In my medicine ward as I earlier said majority of  patients were of chronic liver diseases (CLD) and it includes Hepatitis B, Hepatitis C  and cirrhosis of liver. Cirrhosis is the end result of hepatocellular injury that leads  to both fibrosis and nodular regeneration throughout the liver. The clinical features  result from hepatic cell dysfunction, portosystemic shunting, and portal  hypertension. Cirrhosis may cause no symptoms for long periods. One of the major  complications is uppergastrointestinal tract bleeding which may occur from varices,  portal hypertensive gastropathy, or gastroduodenal ulcer. Hemorrhage may be  massive, resulting in fatal exsanguinations or enencephalopathy. Esophageal varices  are found in 50 % of patients with cirrhosis. There are several treatment and  management options available for esophageal varices includin g acute resuscitation  as initial management, pharmacologic therapy, balloon tube tamponade, portal  decompressive procedures and emergent endoscopy. Endoscopic techniques are also  used for prevention of Rebleeding. 1. Now, I will discuss my case scenario. In our  hospital there is one associate professor who is trained in doing endoscopy and  Wednesday is fixed for performing endoscopies. Emergent endoscopy is performed  after the patient’s hemodynamic status has been appropriately stabilized (usually  within 2-12 hours). Majority of poor patients come to hospital in end stage liver  diseases. I have taken this case series as it is a perfect example of scarce resources. Many patients faced problems as their endoscopies were not performed on time as  there was only one day fixed in a week. Now I will apply the four conditions of  accountability for reasonableness for priority setting and resource allocation. These  four conditions are publicity condition, relevance condition, revisions and appeals  condition and regulative condition. Accountability for reasonableness makes it  possible to educate all stakeholders about the substance of deliberation about fair  decisions under resource constraints. It facilitates social learning about limits. It  connects decision making in healthcare institutions to broader, more fundamental  democratic deliberative processes. 2. In my case scenario I will apply the four  conditions as follows. The first one is publicity condition. It states that decisions  regarding limits to care and their rationales must be publicly accessible to clinicians,  patients, and citizens in a publicly administered system. When the patients suffer  the complication of esophageal varices, they are informed about the limited capacity  of the ward to arrange endoscopy as it is done on only Wednesdays and surgical  ward have their own burden of patients to be done endoscopies, due to this reason  we were unable to send patients to surgical wards and the patients and their  relatives mostly agrees on this setup and if their was any emergency only then we  take help from surgical ward or send the patients to any other hospital, so the first  condition is fulfilled. In above scenario second condition is also fulfilled which isrelevance condition. It states that the reasons for limit-setting decisions will be  reasonable if it appeals to evidence, reason, and principles that are accepted as  relevant by fair-minded people who are disposed to finding mutually justifiable  terms of cooperation. In my case scenario the decision making is according to the  framework. The rationales w ere reasonable as it is evident that we had limited  facility of endoscopy and it was fairly accepted by patients and their relatives and  also by doctors and other hospital staff. In our setup priority was given to those  patients who needed emergency endoscopy rather than those who requires  endoscopy for diagnostic procedures. The third condition is revisions and appeals  condition. This condition is a very common problem in government hospitals and in  our scenario we request consultants from surgical ward to do emergency endoscopy  if we think patient is serious and he or she may die if the endoscopy is not  performed on time or in other case the other hospital is very far so that it will be late  if we send the patient to other setup and here comes the function of oncall  consultants also, the oncall consultants plays huge role in these emergency  situations. This third condition is a mechanism for challenge and dispute resolution regarding limit setting decisions, including the opportunity for revising decisions in  light of further evidence or arguments. 3. Thus we fulfill the third condition also by  revising our decisions as I explained above. The fourth and last condition is  regulative condition or enforcement. There is either voluntary or public regulation  of the process to ensure that conditions 1-3 are met. This condition is also fulfilled in  our setup as we communicate with the patient and their relatives about our limited  resources. We are able to convince patients in our case scenario. The hospital  leadership is constantly making efforts to meet the conditions of ‘accountability for  reasonableness’. 4. CONCLUSION In this essay I have discussed all four conditions of accountability for  reasonableness, for priority setting and resource allocation. ‘Accountability for  reasonableness’ is a framework that can be used to guide legitimate and fair priority  setting in health care organizations, such as hospitals. In our beloved country  Pakistan we have few government civil hospitals bearing the burden of millions of  population. We try our best to server the humanity. Iam not claiming this system a  perfect one, it needs a lot of improvement and the example is my case scenario in  which we have very limited resources. Government should establish civil hospitals in  small cities also and should increase their budget; they should recruit more doctors  and nurses as we have shortage. They should train doctors with latest equipments  and provide hospitals appropriate medicines. In addition to this all the hospitals  should be provided with computers and also be made online so that a data system  can be established and it can help the patients and also hospitals for future  reference. Tobacco contains many harmful chemicals that can damage your heart, lungs, and nervous system. Tobacco companies deliberately put these chemicals in cigarette’s to get us addicted to their products; never less this has been an ongoing controversy for many years. Therefore smoking cigarettes is very addictive and a habit that is very hard to brake.Smoking can cause lung disease, cancer, emphysema, and death. Smoking cigarettes can even cause our skin to look prematurely old and cause wrinkles. People do not realize what harm they are doing to their bodies by inhaling tobacco, therefore should visit with a health professional regarding their options. One very harmful chemical that is in tobacco is carbon monoxide. First carbon monoxide when mixed with other harmful chemicals like tar and inhaled thru the lungs is very dangerous; and as a result can h ave adverse reaction.Consequently researchers have even found in recent years that due to inhaling carbon monoxide thru cigarettes into the lungs can cause carbon monoxide poisoning, as result can lead to stroke or death. Second, harmful effects of carbon monoxide poisoning thru smoking cigarettes is also irreversible. Therefore the damage is permanent and the body will not naturally heal itself. There are prescription medications available today that wil l work with the receptors in your brain to reject the urge to smoke or the desire for nicotine.Another option is counseling and, sometimes talking to a professional can help with addiction. Many people will not quit until they have made up their mind to; or witness someone that they love suffer from a disease such as lung cancer. A person can even visit with their Doctor about trying to quit. Lung cancer and lung disease are the top two illnesses that are the direct effect from smoking cigarettes. Lung cancer is not curable and the mortality rate is higher than those that survive that are diagnosed.It is estimated that more people die from smoking a cigarette than drug and alcohol use combined. Lung disease is a terrible illness to have to endure; some people even have to have their lungs drained to release the fluid that is built up enabling them to breath. More and more people today have to live with the use of oxygen, a device that pumps air into the lungs and helps the lung to f unction normally. Tobacco use is very harmful; we not only put our own bodies in danger but others around us. Changing the world will not happen by introducing people to a formula or giving them ten steps to make their lives better, but by showing them our love. What makes us human is our capacity to love. Love is that quality that erases prejudice and moves us to forgive, gives us peace of mind in a turbulent world, and keeps us sane. This kind of love starts with having compassion. When we live selfishly we have no concern for anyone else. All we care about is pleasing ourselves, and people around us often get hurt. Eventually we start using people and stepping on their toes because it makes us feel better or it advances us in our career or our popularity. We see no problem with using others because it benefits us and we do not even think about how they might feel. We do not care about those who suffer those who starve to death, or the people who are dying because they cannot go see a doctor. We do not think about others because we are too consumed with our own lives, because we have no love for anyone but us. What are the results of being so self-centered? India is the largest democratic country in the world, yet people suffer from depression, pain, and false relationships. People are confused because they can have anything and everything they ever wanted. They work their whole lives to please themselves, but they cannot figure out why that does not satisfy them. People are still left empty and looking for something else. We rarely stop to consider what we could do to make our world a better place. If we tried, we might Just be blessed beyond imagination, and those who suffer would be taken care of. We have to take our eyes off ourselves o see the needs of others and to allow ourselves to feel compassion for them. We must have love. When we love, we will want to reach out to others, build relationships, and make a difference in the world around us. Love can move mountains. It can shake the foundation on which people live. Love can work miracles in the hearts of those who receive it, and we have the power to give it. Love sees broken souls and heals them. Love hears plaintive cries and answers. Love discerns a need and fulfills it. Love does not see color, race or features; love sees people. Just think what we can accomplish if everyone loved unselfishly? Sometimes, because f our experiences in life, we forget how to love but we should always remember that without love we lose the most important part of us. Without each person’s love, the world is poorer and our hearts become, Just an ice box. The power of love knows no boundaries, no walls. Love does not ask for anything in return. It is at times a self-sacrificing gift but it is given freely. For when we give love and see the difference that one act of love can make, the feeling is priceless, indescribable. And in that one moment, we get a glimpse of the power contained in each of us, the the power of our love working in unison, the power to change the world.
